The Surgery
Two major options for augmenting the buttocks are through silicone implants or through injections of your own body fat into the buttock region. Dr. Wong’s preferred method for performing buttock augmentation is fat transfer. In comparison to using silicone implants, fat injection can have a lower risk of infection, achieve a more natural result, and contour the areas around the butt better for an enhanced overall effect.
Buttock augmentation surgery is typically performed as an outpatient surgery where you will get to go home with your family the same day. This surgery typically takes 2 – 4 hours, depending on how much fat is harvested. Dr. Wong performs this in a surgery center or at in a hospital operating room. Areas of unwanted excess fat are targeted and treated with a special solution that has numbing and an agent to decrease blood loss. The excess fat is removed through very small incisions that are approximately a centimeter long. Immediate improvement can be seen once the fat has been removed. The removed fat can be reinjected to augment certain body areas, most commonly, the buttocks and/or breasts. Depending on the patient’s body type, a pleasing hourglass shape can be obtained. The patient will then be placed into a bodysuit or a garment.

After Surgery
Immediately after surgery, patients will feel sore in the areas where liposuction was performed. They will be placed in a body suit (ie. garment), which they will wear for 6 weeks. Blood-tinged drainage from the access incisions can be normal for the first 3 days. Swelling postoperatively begins within the first 24 hours and can become more severe in the next 3-4 days. Prepare to look “black and blue” with bruising, which improves and resolves by the end of the 2nd week. It is very important to stay hydrated by drinking at least 3 liters of water or Gatorade daily for the first week. This will help you recover faster.
If you had fat grafting, do not place any pressure on your fat grafts to avoid ruining their viability for at least 3 weeks. Any excess pressure on the fat grafts can cause your body to reabsorb them. Patients become more and more excited as their results continue to improve with each passing week. Recovery is certainly a process, but be patient and trust the process.
Will I need other surgeries?
For patients with good quality skin, liposuction is all they need for optimal results. For patients who are anticipated to have extra skin, Dr. Wong will discuss additional options to give you the most comprehensive results if this is your goal. Some examples include: abdominoplasty (tummy tuck), brachioplasty (arm lift), mastopexy (breast lift). Fat grafts can get reabsorbed by the body unpredictably and asymmetrically. Rarely, if a patient is desiring an even larger or more dramatic result, they may opt for a second round of fat grafting.
Can have multiple surgeries done at once?
It depends. As a general rule of thumb, this is a possibility as long as you are not spending too much time under general anesthesia, that you are healthy enough for a longer surgery, and that you would not be jeopardizing any of your results by placing constant pressure on the surgical sites. Let Dr. Wong know what your goals are and in what order they are important to you so that she can formulate and personalize your surgical plan.

Note: Should fat grafting be performed, a special closed system is used to process and purify the fat, removing any excess blood cells, oil and fluid. It is important to Dr. Wong that she provides her patients with the most high-tech machines and safest techniques. The special method she uses significantly decreases the risk of infections and abscesses because the fat grafts are not sitting out to allow dust to settle into it and also are not transferred between syringes. Also, it is the safest way to reinject the fat as the built-in pressure gauge does not allow the re-injection pressure to be higher than venous pressure. Given that this is one of this most high-risk surgeries performed by plastic surgeons, it is important that your surgeon has done everything they can to minimize these risks.
